Did you know that Nelson Medical Group has a patient forum that meets 3 to 4 times a year. This gives both the patients and our staff the opportunity to learn from each other about what is happening at the surgery and with the larger CCG patient forum groups. If you would be interested in joining our group please ask at Reception and they will be able to provide you with further information.
If you wish to join our practice please collect the forms from Reception and provide us with two forms of ID these need to be photo (passport, driving licence) and proof of your address (a utility bill or rent agreement). If English is not your first language we can arrange for an interpreter to attend your appointment.
All our patients are provided with a named GP if you wish to find out the name of your GP please contact the surgery.
