General information

Two Counties Care delivers tailored home care services to older people in their own homes and is regulated by the Care Quality Commission (CQC). We are based in Farnham, Surrey and are proud to support members our local community who live in within the GU9, GU10 and GU12 postcode areas. Our services include Personal Care; Light Domestic Duties; Shopping and Social visits as well as taking our Clients for trips out to Garden Centres and other places of interest.

Our team is drawn mainly from the local area and consists of over 30 highly dedicated staff, most of whom have achieved or are working towards formal vocational qualifications in Health & Social Care, from level 2 to level 5. We constantly seek people of all ages who have the right attitude towards caring for people who may be vulnerable.

We pride ourselves on being in touch with the community and strive to find new connections that improve the quality of care we deliver, working closely with our colleagues in Health such as Community Nurses, Occupational Therapists, GPs and our friends across the voluntary sector such as the Farnham Lions.

Our aim is to continually improve the quality of life and well-being of those we care for through investment in a skilled workforce and by tackling issues such as social isolation and unnecessary hospital admissions.

Our workforce is specifically trained in Dementia awareness and in 2014 Two Counties Care established the Farnham Alzheimer Cafe, part of a network of similar Cafes that follow a themed programme of learning whilst providing a safe a friendly social atmosphere for people with memory problems and their families/carers. The Cafe meets from 6pm-9:30pm on the fourth Tuesday of every month at the Weydon Christian Centre in Upper Way, Farnham. It is run by volunteers, including Two Counties care workers, and supported by professionals in Health & Social Care including our local Dementia Navigator.
